Staff meeting documentation did not consistently include the training length.
The following were observed: -A green wooden storage cubby in one room, had a broken slat. -The play kitchen was tippable in one room.

Periodic review of current employees/ volunteers
Bi monthly observations and annual evaluations could not be located for staff working in the program. The Director stated that bi-monthly observations had not occurred since the program reopened in September 2022.
Location of health care policy; Location of 1st aid kit
The program had the health care policy posted in an area that was not visible to parents, visitors and staff. The Director indicated she will make a sign to indicate where the health care policy is slocated.

One room did not have the time of arrival and departure recorded for children for the entire week.
The program did not have an evacuation drill completed for October 2021.

The following were observed on the new projected outdoor play space: --Chipping paint on the door, --a 5 inch gap between the fence and program, --Sand bags that could be a trip hazard. --Vegetation throughout the area. --Screws protruding into the play area. --An art easel leaning against the exterior wall and a coiled hose on the ground.
