1) During a monitoring visit on 9/16/19, educator Gloria was cited for not documenting children's attendance accurately. On 10/8/19, the educator submitted a corrective action plan stating; "Attendance is being taken as children enter and leave the program and I will be sure to have the date on attendance." During this visit on 12/6/2019, the children's attendance was not accurately documented: -there was no attendance log for a private child who has been in care since September 2019. -No arrival time for a privately enrolled childcare child who was present in the program during this visit. The educator did not adhere to the corrective action plan she submitted to EEC in October 2019. 2) The educator has an enrollment freeze since 7/24/19. The educator submitted an enrollment list of 6 children enrolled in her program. The enrollment freeze letter states that the educator must notify EEC when a child dis-enrolls the program and that she cannot replace any child that leaves the program or enroll any new children. During this visit, EEC staff learned that a childcare care child was dis-enrolled from the program in September 2019 and that a new private child was enrolled during the same month (September 2019). The educator failed to notify the Department that a childcare child was dis-enrolled from the program in September 2019. Additionally, the educator enrolled a new child in her program despite her enrollment freeze. 3) Also as part of the enrollment freeze, the educator and her assistant were sent to attend an in-person Infant Safe Sleep training on 09/12/2019, which they both failed to attend.
