During the visit, 4 records, including 3 of the children present were reviewed. The following was observed: 1) Child #1: emergency card and schedule of days and times in care were incomplete in the enrollment packet. The child did not have the general permission completed. Additionally, child did not have medical information (physical, immunizations, and lead screening results) in the file. 2) Child #2: CPR/FA/medical consent was incomplete in the enrollment packet. Additionally, child did not have medical information (physical, immunizations, and lead screening results) in the file. 3) Child #4: emergency contact information was incomplete in the enrollment packet. The child did not have the general permission completed. Additionally, child did not have medical information (physical, immunizations, and lead screening results) in the file.
During the visit, children #1 and #4 did not have completed progress reports in their files.
During the visit, educator did not have her PQR information available.
During the visit, educator did not have attendance records for 2 of the children that were present indicating arrival time for today 11-18-25.
During the visit, and after reviewing the Evacuation Log the following was observed: 1) EEC regulations required to complete a drill at least monthly. No drill was documented for the month of October 2025. , and per educator she forgot to complete one during this month.
During the visit, the following indoor hazards were observed: 1) Large/tall crystal vase on the floor in the living room area posing a tripping and tipping hazard. 2) Mirror on small table and vase decorations on another table posing a tipping hazard.
During the visit, 2 of the children present were in the nap time routine, and 2 of the children, including educators own child were awake upon Licensors arrival. Then the 2 children wake up and all the transitioned to snack time. Per educator working on pumpkins this week. However, educator did not have evidence of a a well-balanced curriculum of specific, planned learning experiences that support the social, emotional, physical, intellectual and language development of all children.
Outdoor Space not approved. During the visit ,Licensor observed the following indoor hazards: 1) an orange extension cord underneath the crib in the playroom area. 2) speaker on stand in the living room areas presenting a tipping hazard for children. 3) electric cable on the floor in the living room area presenting a tripping hazard for children. 4) body and feminine wash in the edge of the tub in the bathroom area.
During the visit, Licensor observed the following: -CHILD #1: enrollment packet did not have date of admission, physician information, written consent for general permission and CPR/FA/medical consent. Additionally, the developmental history and emergency card were incomplete in the enrollment packet. In addition, the child did not have medical information (physical examination, immunizations). -CHILD #2: was present and arrived at the program during the visit. However educator did not have an enrollment packet or medical information (physical examination, immunizations, and lead screening results) for child.
