After reviewing the evacuation Log. The following was observed: 1) EEC regulations require to alternate emergency exits monthly. Educator has 2 separate exits , however the evacuation log showed that only the " front exit" was used for practice drills completed during the months of February to July 2025. 2) The evacuation log did not have EFFECTIVENESS of practice drills documented the months of February to July 2025.

The educator requested to ad the outdoor space. During the visit, Licensor observed a structure with slide attached (22 inches) that measured more than 18 inches in height and educator did not have a Fallzone underneath equipment with an adequate dept of an impact absorbent material as require.

The educator requested to add the outdoor space (enclosed side yard and backyard). During the visit , the following hazards were observed outside: 1) Backyard area: - plastic pool handing from the fence -loose orange electric cord that was -roll of electric cable on the ground posing a tripping hazard -electric foot machine posing a tripping hazard -pole on the ground posing a tripping hazard -the gate (door) did not closed properly and did not had a mechanism to lock it and prevent children from accessing the street - 2 hanging ropes that were low which educator said were used for drying clothes -a latter -tools (shovels)
