During the visit ,Licensor observed an {REDACTED} months sleeping inside a playpen upon Licensor's arrival. The following was observed: 1) EEC policy states that Blankets, comforters, pillows, stuffed animals, wedges, positioners, bumper pads or other soft padded materials or toys must not be placed in the crib with the baby. The {REDACTED} was observed sleeping with two different types of toys (teething toys), including a soft toy inside the playpen. 2) EEC policy states that Jewelry of any kind must be removed prior to placing a child to sleep, unless the child’s parent has given the program written consent to leave jewelry on during sleep. During the visit, the {REDACTED} observed had stud earrings while sleeping, and educator did not have a written consent from the parent as required. 3) EEC policy states that Infants must be placed on a firm, flat, non-inclined sleep surface; and Pillows, cushions, and mattress toppers designed to make the sleep surface softer must not be used as substitutes for mattresses or in addition to a mattress. During the visit, educator had an additional mattress that was added inside the playpen where the infant was sleeping. 4) EEC policy states that the use of soft objects or loose bedding in and around the crib is prohibited. During the visit, it was observed that the playpen mattress observed underneath the other mattress that was on top, was covered with a plush blanket instead of a fitted sheet as required.
