Per educator the assistant was attending a medical appointment. However, Licensor observed educator was not accurately documenting attendance as the attendance log for assistant showed the assistant sign out time as 4:30 PM for today 6-5-25, and the attendance for the following day (6/6/25) as well as next week was already completed showing assistant sign and out times (6:30 AM to 4:30 PM) from the program.
