Program Director provided evidence of Staff Meeting dated in 2022. Only one Professional Development documentation from 10/14/2024 was available for review.
During a review of all Staff Files, it was noted that the new hires did not have complete files as required by EEC. At least 10 staff files did not have evidence of the following: Staff Orientation, a Current Physical, 2 MMR’s, BRC suitability letters in the personnel files, EEC Essentials 2.0 for new hires prior to working in the classroom with children, at least two staff that were hired over 6 months ago and had no evidence of 1st Aid,
During the file review, over 8 new employees did not have evidence of a suitable BRC in their personnel file. During the visit the Director searched in the BRC Navigator to provide evidence for some employees.
In the toddler classroom, several learning areas appeared sparse with inadequate curriculum materials for the number of children present. The dramatic play areas/kitchen units were empty in one area there was a small number of toy foods. The program director did show the licensor consumables that have recently been purchased, however the licensor did not observe newly purchased toys for the children.
During the renewal visit, the licensor observed clutter on top of shelves and cubbies in several classrooms. The observations included: Several Dunkin Donut iced drinks and food bags on shelves, at least three purses were on top of shelves along with bottles sippy cups and other items.
Staff Schedule was not posted in a manner in which parents, visitors and EEC could review. The schedule does not indicate educator’s assigned classrooms, break coverage, First Aid and CPR training, or certification.
Allergy Posting was not available for review in several classrooms. In two rooms educators provided the licensor with some allergy information, however they reported that the logs were no longer current and many of the children were no longer in the program.
In the Pre-K classroom and in toddlers the evacuation procedures and diagrams were not posted near the exits.
Educators report that they were unaware of one child’s IHCP. The child has a diagnosed allergy that requires an epi-pin. During a review of the child’s file, there was no IHCP on file.
One child’s epi-pen was expired during the visit.
Staff and children did not wash hands after diapering. Three diaper changes were observed. During all three diaper changes, the educator failed to wash their hands after changing the children’s diapers. The educator changed gloves between diaper changes but did not wash their hands. The children were not encouraged or assisted to wash their hands either.

During an inspection of the classroom, some areas of the walls where posters and decorations have been removed have chipped or peeling paint.
During the review of one child’s IHCP, it was noted that the plan expired in 5/2022, the allergy action plan expired in 7/2022 and the medication consent forms expired in 5/2022.

During a review of four staff files, three files did not have evidence of physical forms or two MMR’s.
During a review of the Educator files, there was not evidence of a qualified administrator.
During a review of 4 children’s files, at least one file did not have evidence of a developmental history, transportation plan, and face sheet. Three of the four files reviewed had evidence of expired physical forms and one file did not have a physical.
During the “Safe Sleep” observation, One child’s crib had a loose fitting sheet, that was provided by the parent.

During of a review of the evacuation procedures it was noted that the diagram is posted, but the evacuation procedure is not.

Observations from original visit on 2022-10-20 00:00:00: An older infant was being fed from the serving dish and with the serving spoon used to distribute food to all children. Corrective Plan received on 2022-10-25 00:00:00: Staff have been retrained on feeding within the center for all classrooms and new infant bowls and spoons have been repositioned within the classroom for easier access. Children will all have single serve bowls for snacks and meals. 2022-11-09 - A bottle warmer was observed in the infant room and was being used to warm an infants bottle.

Materials are scarce in most classrooms. In toddler rooms very few materials were observed
2 infants were observed sleeping on their backs, wrapped in blankets in their cribs.
Infant child with lots of mucus draining from nose, had the xylophone hammer in his mouth and began crawling, causing a potential choking hazard. Once licensor pointed this out, child was picked up. The xylophone remained on the floor and another infant put the hammer in his mouth.
An older infant was being fed from the serving dish and with the serving spoon used to distribute food to all children.
Lights in the infant rooms were off while children were crawling about, making it very hard to see in the room. Multiple bathrooms have plungers next to toilets, swiffers, and brooms. May lead to cross- contamination.
There was clutter on shelves in both infant classrooms. Empty baby bottles, purses, adult juices, and many other items were cluttering the space.
Nasal spray for child in the First Aid bag. The meds are not stored properly with script or box there is no medical information present. Infant rescue medication is kept in clothing drawer with out medical forms or parental consents
In Pre-K, an incomplete IHCP is located with medication, also their is no parental consent form. Pre-k room there is a child’s nasal spray in the First Aid kit bag, with no name or prescription label affixed or IHCP

There is a door located on the preschool playground that is missing a metal piece and poses a risk to children as it is sharp. If there are any other pieces that breakaway from that door it could potentially become a head entrapment concern as well.
There was an epi pen missing in the infant room and also there was only one epi pen for a child enrolled in the toddler room.

During the visit the licensor was observing in a preschool classroom and there was a child who did not want to join in which is okay, however the child was balancing on a chair and the cubbies and the teachers did not address it.
Classroom floors, baseboards, and vents (that were in the reach of children) were not clean. There were toilet brushes and plungers accessible to the children at the time of the visit. In the infant room there was baseboard missing from an area and there was a large gap between the floor and the wall that could pose a risk to children.
Some of the exits were missing evacuation procedures and maps.

After a staff member cleaned a diaper table they did not wash their hands.

During the visit the outdoor play space had three areas of entrapment concerns. 1. Toddler playground bottom of the fence where it meets the ground. 2. The gate between the toddler playground and preschool play ground was not even and was an entrapment, and 3. the fence on the preschool playground where the HVAC equipment is stored is an entrapment hazard.

During the review of children's records progress reports were missing, dates were missing to know if paper work was up to date, physicals were missing LEAD documentation, and paper work was out of date.
Children in the preschool classroom were arguing with each other, throwing things at each other and educators were unaware of these situations, children were approaching the licensor for help.
There were not enough materials in the preschool room.
At the time of the visit there was expired food in the programs freezer.
At the time of the visit the fire inspection was expired.
In the infant room there were sharp objects in the wall under the mirror.
