Space (health risks)
Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The following hazards were observed: ** LIVINGROOM **- outlet not covered ** DININGROOM **- no hazards ** BEDROOM **- outlet not covered ** PLAYROOM **- no hazards ** KITCHEN **- drawer to the right of the stove had small chokable items inside, drawer on the far left also had small chokable items ** MUDROOM **- no hazards ** BATHROOM **- no hazards ** OUTSIDE **- red rock and green rock are right next to the fence and a child could climb on top of the rock and then over the fence, small trampoline in the fenced in area is prohibited in childcare and needs to be removed.
Educator does not have a diapering surface. She stated she uses a disposable layer but had run out of them and could not show Licensor what it is she uses.

Licensor reviewed 2 of the children's records. Neither of the records were complete. The 1st had no signature on pg. 6 of the enrollment packet, under basic transport. The 2nd had an out of date physical and no evidence of lead testing. Educator also has a grandchild that was in the home the day of the visit and she does not have a record for him.
Educator initially stated she did not complete the EEC Essentials 2.0 training yet. She then stated she thinks she did complete the training. However, Educator was not able to produce the training certificate to verify that she completed it.
Educator had a 4 month old infant in a pack and play throughout the entire time that Educator was in the home and did not check on the child once.
There is 1 child in the program with a[REDACTED] who is prescribed [REDACTED] This information is posted in the program, as is required by regulations. However, Educator did not have a cover sheet over the paperwork to protect the child's confidentiality.
Educator does not transport the childcare children in her vehicle. However, she does go for walks with the children. There is 1 child whose parents signed that they do not want their child taken off the premises, in the Basis Transport section of the enrollment packet. There is another child whose parents did not sign any part of the Basic Transport section, so Educator also does not have permission to take that child off the premises.
Educator stated she does not have a first aid kit. She reported it was left outside and got wet. Tested the combination smoke and carbon monoxide detectors that are hard wired throughout the home, and they worked properly.
Licensor was in the program for the monitoring visit for over an hour. During that entire time, Educator kept a 4 month old Infant in a pack and play, Educator also kept a 10 month old infant in a booster seat.
For curriculum, Educator stated the daily schedule includes reading, outdoor time, lunch, nap time, and then more outdoor time. Educator did not speak of doing any work with letters, numbers, colors, shapes, etc. There was no evidence in the program of any curriculum being completed.
UPDATE FROM VISIT ON 10/24/24: ** The paint on the rear deck has been scraped and the deck has been stained, eliminating the peeling paint. Educator also has installed a fence around the outdoor playspace in the back yard. However, there is 1 piece of fencing that is raised up and the gap between the bottom of the fence and the ground has created an entrapment hazard. Also, there is a large rock (taller than the fencing) that has a ladder leaning against it. If a child were to climb the ladder to the top of the rock, they would be able to jump over the fence to the other side. This is a hazard and a danger.

Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The following hazards were observed: **LIVINGROOM** - outlets not covered **DININGROOM** - outlet not covered **BEDROOM** - outlets not covered **PLAYROOM** - outlets not covered **KITCHEN** - sharpies, beaded necklace, cords in the drawer to the right of the stove, and similar hazards in the left most drawer. **MUDROOM** - no hazards **BATHROOM** - no hazards **OUTSIDE** - peeling paint on small porch and also on larger deck and railing in the back of the home. The fencing that is currently in place in the back yard is insufficient as it is not 4ft high and it does not extent on all sides, so there is no truly enclosed area. Until an approved fence is put in place, the back yard is not approved outdoor space.

The first aid kit was missing scissors and medical tape. There was also no emergency contact information for the children in the kit.
There are currently 3 children enrolled in the program. Of these, 2 progress reports were overdue, and the 3rd report was not due yet because the child recently enrolled.
In both 2021 and 2022, Educator completed only 6 hours of professional development. This year, Educator completed 11 hours . Already scheduled for another training to make up the hours. However, at this time, Educator has only completed 23 hours of professional development over the last 3 years. Discussed that regulations state 10 hours of trainings are required to be completed every year.

Licensor reviewed 3 of the children's records. Of these, 1 was complete and up to date, and the other 2 do not have any permissions signed in the enrollment packet.
Educator has an up to date Evacuation log. The most recent fire drill was conducted on 8/31/23. However, Educator has not been recording the length of time the drill took to complete.
Educator does not brush the children's teeth. The signed Oral Health Non-Participation forms need to be updated.
There is a child who just started in the program who has allergies and is prescribed an [REDACTED], but Educator does not have IHCP for the child.
All of the required items were observed in the first aid kit, except a roll of gauze and medical tape. Educator also does not have an EpiPen in the first aid kit for the child with allergies.

Licensor discussed with Educator the need for a list to be posted of all emergency or lifesaving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications that specifies to which child they belong. Licensor also discussed that this information needs to have a cover sheet in order to protect the child’s confidentiality. Right now, there is a child enrolled in the program who is prescribed an EpiPen due to allergies. This information is not posted.

The following hazards were observed: **LIVINGROOM - no hazards ** DININGROOM - no hazards ** MUDROOM - no hazards ** PLAYROOM - no hazards ** KITCHEN - no hazards ** BEDROOM - outlets not covered ** BATHROOM - no hazards ** OUTSIDE - needs a different fence and to make sure all fencing is 4 ft high.
For diapering, Educator uses a nap pad with a sheet covering it. Discussed the need to remove the sheet as it can not be disinfected after each use. Also discussed the proper process for diapering, according to the regulations.
Napping materials, such as blankets, need to be stored individually so they are not touching each other, in order to avoid cross contamination.
No immediate health risks were observed during the monitoring visit. There were a couple of items missing from the First aid kit, including medical tape and tweezers. Tested the combination smoke and carbon monoxide detectors that are hard wired throughout the home and they worked appropriately.

Educator lives in close proximity to Rt. 2 so this is a hazard. Trampoline in back yard which the children have access to (ramp).

Licensor was able to inspect all of the licensed space during the visit and observed the following hazards: ** LIVINGROOM - no hazards ** DININGROOM - no hazards ** MUDROOM - no hazards ** PLAYROOM - no hazards ** KITCHEN - drawer to the right of the stove has lock that’s not working and there are hazardous items inside, knife block on the counter. ** BEDROOM - no hazards ** BATHROOM - hygiene products on the floor of the shower, which children have access to. ** OUTSIDE - playset does not have an adequate fall zone. Educator needs to block access to it so that the children cannot climb any part of the playset. There is a trampoline in the backyard and Educator is aware that she also has to ensure the children cannot access this equipment.

Licensor reviewed 3 of the children's records during the virtual visit. One of these records was complete and up to date. The other 2 records were both missing a signature under the Basic Transport section of the enrollment packet. Please review all of the children's records and ensure they all have this signature.
The fist aid kit is missing tweezers, medical tape, and did not have the children's emergency contact info in it. Please ensure that these items are added.

The following hazards were observed: ** Kitchen - knife block on the counter needs to be made inaccessible. ** Bedroom - small trampoline is a hazard and needs to be removed. ** Living Room - no hazards ** Dining Room - no hazards ** Mudroom - no hazards ** Bathroom - no hazards.
Children's Records - R Apr 2025
There are 6 children currently enrolled in the program. Educator stated [REDACTED]. All 6 of the enrolled children are under [REDACTED]. On any given day, Educator has either 4 or 5 children under [REDACTED] in attendance. Licensor made it clear that this is against regulations as there can only be a maximum of 3 children present under 2 years of age, and 1 of those children has to be at least 15 months old and walking.
Educator could not locate the Evacuation Logs (fire drills) during the virtual visit. She stated she would email them to Licensor, but Licensor did not receive any email.
Educator tracks attendance on her computer and puts the time of arrival and departure for each child daily. She then prints the forms individually for each child. Licensor expressed concern around this method of tracking attendance as Educator simply has the same time written in for each day, with no variation. There was also one day when a child was dropped off, but then Educator was informed the child had [REDACTED] so she had the parents pick up the child. However, in the child's attendance, the child was marked as being present for the entire day. This method of tracking attendance does not seem to be entirely accurate.

Educator has installed a new play structure in the back yard. The fallzone is not sufficient as it does not extend 6 ft out in every direction. The fallzone will need to be completed before the childcare children can use the play structure. There is a trampoline in the yard. Educator stated it is inaccessible as she has removed the ladder.
Most cleaners are stored under the sink in the kitchen, in a locked cabinet. Educator also has a bucket on the kitchen counter with additional cleaners that she uses constantly. Discussed that this bucket needs to be moved from the counter as this is considered accessible to the children.
