Individual Health Care Plans and medication consent forms for chronic medical conditions were missing from some children's files.
The program did not maintain appropriate staff child ratio when one educator was alone with 18 children in the outdoor space.
It was observed that the floor has broken tiles in the school age room, one shelf in the infant/toddler room is tippable, a sink needs repair in the children's bathroom, some heating vents are dusty and dirty, and a door leading to a public space was unlocked.
The fall zones under and around the climbing structure does not the meet the required depth of 9 inches of impact absorbing material.

During the inspection of the outdoor playground, it was found that the chain link fence had sharp edges along the bottom of the fence and needs repair, and that the bottom of the poles supporting the shade structure had exposed nails and needs covering.

a Random review of children's records revealed that two Individual Health Care Plans did not include information on who trained staff on the medical conditions.
On the day of the virtual visit the first aid kit did not include non-latex gloves, gauze roller bandage, tape, or tweezers.
There was chipping and peeling paint on the exterior of at least two classroom doors used by children to enter the facility.
