Progress Reports
Random review of four children's files. Three children need new enrollment packets or annual update forms signed by the parents. Two children need current physical and immunization documentation. One child had no physical or immunizations on file.
Infant/toddler and preschool progress reports reviewed were completed in September 2024 and November 2025. Once child did not have any progress reports in their file and is due. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).

Educator does not currently have 30 hours of professional development training certificates for this licensing cycle. 10 hours of training are required per year, 1/3 must focus on diverse learners. Educator will email the licensor copies of the certificates when completed.

Front door and back door in the kitchen. The back exit was not shoveled. Exits must be accessible and safe at all times.
Indoor Hazards
Basket behind the toilet is accessible to the children and contains hazards such as sprays, mouthwash, etc.

Attendance is tracked using the Guild of St Agnes sheet for Guild children and a separate sign in sheet for private pay children. Discussed making sure that the attendance is accurate at any given point in the day with the daily arrival and departure times of the children recorded. Some children were not signed in at the time of visit.
Large electric fan was accessible in the playroom. Trash can in the bathroom did not have a lid. Lawn mower was out and accessible in children's play space. A sensory table was filled in standing water and leaves from recent rain. The cover was next to it on the ground. Discussed ensuring the table is covered.

Reviewed four children's files. One child was missing medical documentation.
Mobile first aid kit. Emergency card pages for the children are current. CPR mouth guard or shield is missing.
The refrigerator thermometer was not working at the time of the visit. Temperature must always be between 32 and 40 degrees. Educator contacts with the Yours for Children food program.
This follow-up visit inspection recorded no violations or advisories.
