Fall zones adequate depth of impact absorbing material
One child enrolled in the program with an allergy that requires an Epi-pen and Benadryl did not have an Individual Health Care Plan on file
An inspection of the out door playground revealed that the fall zones under and around the climbers and swings did not have the required 9 inch depth of impact absorbing material.
Evacuation procedures posted at each exit
Children began arriving at 3:00pm ten minutes later Educators were trying to record children’s arrival and they did not know how many children were present. When they were done the attendance record was missing a child that left the cafeteria to go to the bathroom.
Emergency evacuation procedures were not posted near exit,
On the day of the visit Educators did not know the location of the building and fire inspection and were not sure if they were current.

The program did not have a staff schedule posted and Educators were confused as to who the third staff person on duty was.
Emergency evacuation procedures were not posted near exits.
One child with an (REDACTED) did not have an IHCP. Another child with (REDACTED) did have and IHCP for the (REDACTED) condition.
On the day of the visit the program did not have a supply of snacks available for children who’s parents may have forgotten to provide a snack.
Upon arriving to the program children were not encouraged to wash hands before they had snack. Many of the children had snack without washing their hands.
At least one child with an (REDACTED) did not have parental authorization for the administration of the (REDACTED)
