Indoor Space
During the visit the following was observed in the indoor space, fans accessible to children. Diapering tables were covered with sheets in two classrooms and not changed between children. There were cords accessible to children in toddler 3 longer than 6 inches. There were cubbies outside of toddler 1 that were not secure and tippable.

During the visit there was a pair of scissors on the changing table. The changing pad had a tear in toddler two and the rugs in toddler one were curling up which could pose a tripping hazard. The rugs in one of the rooms were dirty and should be cleaned or replaced.
The new outdoor play space for infants has head entrapment concerns around the bottom of the fence, there was mulch and small rocks accessible to children that could pose a choking hazard. Part of the ground was uneven with two tree stumps that could pose a tripping hazard to infants especially those learning to walk.

During the visit an educator was observed washing tables, chairs and a trash can and never washed their hands after the cleaning tasks.
There is no sink next to the diaper table in the infant room, one toddler room also does not have a sink next to running water.
At the time of the visit the staff schedule was not current.

During the visit it was observed on two different occasions hands being washed with sanitizer. One time a child did not wash their hands after a diaper change and the educator only washed with sanitizer. During the visit it was also observed that children and educators did not use soap and water to wash their hands before sitting down to eat lunch. The educators and children used hand sanitizer to wash hands instead. After a diaper change the changing table was not cleaned and disinfected.

At the time of the visit there was a bottle warmer in the infant room. A shelf in the infant room was not secured and could pose a risk to children. The trash was not covered in the toddler bathroom.

There was a musical instrument, a tambourine stick, that was not developmentally appropriate for Infants. Furniture and shelving units throughout the facility were not secured from tipping. Linoleum used under tables was buckling creating area of tripping hazards. The linoleum under the table in Toddler 1 room was not secured and could also create a tripping hazard for toddler children.
One crib used as an Emergnecy crib did not appear to have reinforcement or adequate wheels for the purpose of evacuation.

The program did not have a staff records checklist available. Staff record for one Educator did not include the following: staff from, physical, evidence of MMR vaccine, BRC, or evidence of Interview questions.

Evacuation procedures posted at each exit

Emergency evacuation procedures were not visibly posed near the exit in the Infant room or in the Toddler 2 Room. There was no diagram of evacuation routes posted near an exit in Toddler 2 Room that did not lead directly outdoors.
The program did not have a current children’s records checklist on the day of the visit. A review of children’s records revealed that some files had not been updated since 2017/2018

Emergency Evacuation - Cribs
Through interview it was learned that a (REDACTED) regularly spends time in the preschool room each day while waiting for the Educator to get off work. The (REDACTED)was observed interacting with children and helping to put away nap time supplies. It was learned that the (REDACTED)did not have BRC information available for review. A review of staff records revealed that at least one Educator also did not have BRC Information on file.
A toilet booster seat was stored accessible and within reach of children. On the day of the visit there were dishes sorted to dry in the bathroom used by children.
Tooth brushes were stored touching each other.
There was peeling paint throughout the facility on the day of the visit.
The infant Room refrigerator did not have a method of monitoring the refrigeration of food
