Licensor randomly observed 4 children records. Licensor observed 2 records are outdated.
Licensor observed the record for every child enrolled and te following was noted: Child #1- missing physician/health care Child #2- missing physical description and physician.health care, section for CPR/1st aid is empty Child #3- missing physician/health Child #4- missing physician/health, parental visit notice is empty. Child #5- missing physician/health Child #6- incomplete physical description, missing physician/health care, sections for acknowledgement of parent handbook are empty. All children records are missing certified assistants information
Although fire drills are documented monthly, they’re only happening in the morning time.
Licensor observed first aid kit and it was missing emergency card information for all the children.
Although Licensor observed a parent handbook it must be updated since it indicates that Educator does not work with an assistant and does not specify exits in emergency plan section.
Educator has a cat (although it was not observed during the visit). Vaccine record does not indicate date for rabies vaccine.
