The educator did not have current children’s progress reports for 3 child’s records review. The educator is not conducting progress reports in the frequency as required.
One assistant record had expired physical.
1. The educator has a gallon of Clorox, a box of Borax detergent and 2 additional gallons of laundry detergent accessible to children on the dining room floor. 2. There were toaster cables accessible to children in the dining room.
Upon arrival to the program this licensor observed that the educator was using an unlicensed room for napping children.
1. At the time of this visit, the licensor observed that the educator had a {REDACTED} and a {REDACTED} month old {REDACTED} sleeping on an {REDACTED} seat with a fleece blanket in the living room. 2. The educator did not have playpens available for the {REDACTED} to sleep in. The playpens were stored away.
The licensor observed that the educator was not following diapering procedures when she was changing children diapers and she did not wash the children's hands, nor she disinfected the diapering surface and she did not wash her own hands between diaper changes.

During the November 21, 2019 visit to the program the licensor learned that the educator was out doing errands with 2 daycare children. During this visit, the educator shared that she has taken the children to the mall to play on available rides there, for which the educator was not providing age appropriate activities with the children.
One smoke alarm in the daycare room was not working.
Four children’s records reviewed did not have the assistants’ names on the general permissions and the parental authorization to administer first aid and CPR.
The educator acknowledged been transporting children in her private car to the mall. The educator did not parental authorization to transport children in her private car.
