Licensor observed bathroom locks in the drawers were not working TV in the nap room is not secured.
Educator requested to have backyard added to licensed space. There is a small gazebo situated atop a platform 1 foot 8 inches high, which the educator requested permission to use with the children. Beneath the platform, there is a gap that requires protective netting to prevent children from falling into it. Accessing the platform requires climbing three steps. No pressure gate was installed at the bottom of the stairs.

Indoor Hazards
The licensor observed that the handrail spaces in the stairs are large enough to fit a child. Also, at the bottom of the indoor stairs a barrier must be installed since the program serves young children. The licensor observed some lose hanging printer cables connected to the receptacle . There is a box of an air conditioner in the blocking the entrance.

During the visit, Licensor reviewed 3 records of the children that were present. The following was observed: 1. Three children were missing certified assistant J.H information on the General permission and CPR/FA medical consent release in the enrollment packet. 2. A child’s physical examination was not up to date on file.
During the visit, Licensor observed two unprotected radiators (living room area and bathroom located near the kitchen) by means of permanent screens, protectors, insulation or other suitable device that prevents children from coming into contact with them.
During the visit, Licensor observed a large shelf that was obstructing the evacuation route of the exit located through the front of the home.
No carbon monoxide was available.
Licensor observed under the sink drawers were unlocked and contained beauty products and cleaning products.
Emergency preparedness plan must update location for monthly fire drills.
