The licensor reviewed four different children records and observed the following: 1) Child #1 does not have a lead test on file. 2) Child #1, Child#2, Child #3,Child #4 did not have certified assistants names on the general permissions forms.

The licensor observed that attendance sheets for children were not fully completed for the period of 1/26/2026 through 1/30/2026. Documentation was only completed for one child out of the ten enrolled in childcare.

The licensor reviewed four different progress reports and found that child #3 was missing a progress report.
The licensor observed that the allergy form for the child enrolled with an egg white allergy was not posted or readily accessible at the time of the visit.
The licensor observed that when the educator was passing out snacks, the children's hands were not washed prior to snack service.
The educator has a child enrolled with an egg white allergy and is required to maintain an Individualized Health Care Plan (IHCP) and ensure that the prescribed EpiPen is available and accessible at the childcare program at all times.
The water temperature was 122*F at the time of the visit.
The licensor observed that the educator did not have evidence of a plan describing how program activities support and engage children through specific learning experiences. Such plan must be appropriate to the ages and development of the children served, to the length of the program day and to the program objectives. As appropriate, children must participate in the development of the plan.
