At the time of the visit, the licensor reviewed a sample packet of children records and observed the following: 1)Child#1 has an expired physical report and immunization report dated from 3/24/2025. And is missing a lead report test in child file. 2) Child #2 has an expired physical report and immunization list dated from 7/25/2024.

The licensor reviewed a sample packet of progress reports and observed the following: 1)Child #1 is missing a progress report in child file.
At the time of the visit, the licensor reviewed the emergency evacuation log and observed that for the last two months has been completing the evacuation logs in the afternoon at 3PM. And for the month of February and March the educaor completed the emergency evacuation drills at 10AM. Also, the smoke detector log was empty and did not have any information documented.
