Licensor randomly observed 12 records. A list with missing items for each record was handed to Educator.
Licensor observed 12 children records. 1 child is missing progress report.
CA-6 1. Missing ID 2. No professional development for 10.30.23 to 10.29.24; and from 10.30.24 to present. (Educator needs to complete 10 hours of professional development). RA 1. No ID.
Although there’s a parent handbook, it does not include Assistants information.
FA kit has an outdated emergency card.
Educator did not have evidence of having completed 10 hours of professional development for licensing year 3.2.22 to 3.1.23. Educator needs to complete 1 hour of professional development in order to have 30 hours in total.
This follow-up visit inspection recorded no violations or advisories.
At the time of the visit, this Licensor observed a {REDACTED} taking a nap in a playpen. The playpen had a fitted sheet that was too big for it (loose) and in addition, between the child and the fitted sheet, there was a fluffy sheet, which the Educator indicated to put so that the {REDACTED} would be more comfortable.

Licensor observed: dramatic area plastic kitchen, 1 tambourine, 1 toy piano, 1 cash register, 1 electronic board game and 2 toys for infants. This is not sufficient amount of toys for licensed capacity. In addition, Educator is requesting a capacity upgrade, needs toys for school age children.

Licensor observed the record for every child enorolled and the following was found: Child #1- missing IHCP for a diagnosed allergy and {REDACTED}. Child #2- missing physical exam, immunizations and {REDACTED}. Child #3- emergency card information is incomplete. Missing emergency contact in intake. Child #4- missing schedule of days/times in care, emergency card information is incomplete. Child #5- missing {REDACTED} Child #6- missing schedule of days/times in care and {REDACTED}.
Parent Handbook
Educator uses a parent handbook created by herself. Licensor observed it was missing the following information: 1. Does not indicate where are the emergency exits/door in the program. 2. Missing child plan is incomplete. 3. Section for evacuation plan is incomplete.
Licensor observed a playpen located in front of the kitchen exit making it inaccesible.
The record for every child enrolled was observed and the following was noted: Child #1- LEAD and physical are outdated (Exp. 1-31-24). Permission section is neither approving nor declining. Child #2- ok Child #3- ok Child #4-According to parent, child has a food allergy, but physical exam on file does not indicate any allergy. Child #5- Missing emergency contacts. Incomplete developmental history. Child #6- Permission section is neither approving nor declining. All children are missing assistants information on permission section. In addition, Educator stated that she was caring for two private children during this week since 2 subsidized were going to be absent and theres no record/paperwork for private children.

Equipment and Materials
Educator has 6 children present at the time of the visit. Attendance shows 4 children present.
Emergency cards are not dated.
Licensor did not observed training hours for 2022. Educator has more than ten hours of professional development for 2023.
Findings from last years visit not corrected. In addition, Educator is working with two assistants and parent handbook indicates that there are no assistants in the program.
Information for a child with food allergy (according to parent) is not posted.
Indoor hazards: 1. Uncovered trashcan in the kitchen. 2. Uncovered outlet in the bathroom. 3. Water temperature was 130*F, exceeding the limit of 120*F.
There’s no evidence of curriculum planning. Educator purchases Melius curriculum and there are childrens artwork in the wall but are not dated and have the name of children that left the program. In addition, TV was on during the whole visit.
Licensor observed Educator changing diapers and placing the wet ones in a hanging bag in the diaper table. In addition, children did not was their hands after the diaper change.

Last fire drill documented is from May 20, 2022. Last battery check documented is from March 2022.
Educator observed there are no progress report for children enrolled.
A child with a diagnosed allergy was not listed in the emergency list.
Although there’s an emergency preparedness plan, the following was observed: 1. Meeting for monthly fire drills is 15 minutes away walking. 2. Lock down procedure does not indicate location of the lock down room. In addition to this, Licensor observed that last fire drill documented was from 5/20/22 and that batteries were last revised on March 2022, according to documentation.
Licensor observed first aid kit bag and it was missing the following: 1. Thermometer 2. Emergency cards 3. CPR mouth guard
Educator did not have evidence of taking trainings this past year.
Upon Licensors arrival a child was napping in a bedroom with door closed.
Licensor observed water temperature was 126.5*F exceeding the 120*F stated in the regulations.
Licensor observed the following safety hazards: 1. Bathroom- bar soaps (Coaba soap) opened and accessible to children on top of the toilet tank. 2. Bathroom- laundry detergents opened and accessible to children in the bathroom floor.
