ALthough Licensor observed a current progress report on file, they showed no evidence of having them discussed with parents.

During the visit, Licensor reviewed the Children's records checklist completed by the educator, and the five records of the children currently attending care, Licensor observed the following: 1) three children enrollment packets were not up to date. 2) a child was missing {REDACTED} results. 3) a child physical exam was not up to date. 4) a child was missing a progress report.
During the visit, Licensor observed two lose cords in the new added {REDACTED} room accessible to the children, which were longer than six inches and were not part of recreational or educational material.
