Licensor observed the record for every child enerolled and the following was noted: 1. Child #1- incomplete physical description and developmental history. Child is of school age and school ostentation is not filled out. Child #2- incomplete pohysician information and developmental history. Parent did not sign/date the general permission section. Child is of school age and school information section is not filled your.
Licensor observed the following health hazards: 1. water temperature was 13*F, exceeding the limit of 120*F. 2. Charging cable having loose in the daycare room 3. Broken shade in the daycare room
1. Materials observed: Barbie doll house, dramatic play kitchen, library, coloring books, Lego blocks, 1 puzzle missing pieces, 1 painting brush, 2 children/1 adult sized scissors, 1 small box of paints, white paper, toys inside a bin. This materials are not sufficient for a license capacity of 6. 2. Some materials were not accessible to children. For exameple, Educator hd to get the only brush available. Some other art materials were stored inside a china cabinet. Educator had a large bin with lots of toys inside.

Radiator cover in the daycare room was observed to not be permanently installed.
1. Water temperature was 150*F, exceeding the limit of 120*F. 2. Licensor observed 1 broken shade in the living room and another one in the daycare room.
First aid kit was missing tweezers and emergency numbers. Observations from original visit on 2024-02-16 00:00:00: Licensor observed first aid kit and the following was observed:: 3 outdated portable emergency contacts, no portable emergency numbers, missing: tape, bandaids, gauze pads, instant cold pack, scissors and tweezers. Corrective Plan received on 2024-02-21 00:00:00: I bought a new first aid kit, and I'll also put the emergency contact inside the first aid kit.

Emergency plan posted was outdated. Reminded Educator to use the form that was provided to her during the visit of 2/16/24. Licensor and Home Visitor explained how to complete it. Observations from original visit on 2024-02-16 00:00:00: Educator could not find emergency preparedness plan. Licensor provided template in Spanish to meet this requirement once it is completed. Corrective Plan received on 2024-02-21 00:00:00: The emergency preparedness plan is posted on my childcare's blackboard.
Educator stated that she changes diaper in the bed in the bedroom, which is not part of licensed areas.
During the visit, Educator stated that she takes children to the park or stores in her vehicle. Educator did not have a written plan for this. In addition, permission sections in children record does not authorize educator to take children out of the program in a vehicle, only by walking.

Last fire drill documented is from 2/8/24. Educator stated that she did no fire drill for January 2024.
Licensor observed the record for every child enrolled and the following was noted: Child #1- physical and lead screening are outdated (1/3/23) Child #3- missing transportation plan for pick-up.
Educator could not find emergency preparedness plan. Licensor provided template in Spanish to meet this requirement once it is completed.

Licensor observed the following indoor hazards: 1. Uncovered trash can with trash in it. 2. Water temperature was 135*F, exceeding the limit of 120*F. 3. Educator stated that she’s changing diapers in a bedroom that is not part of licensed areas. 4. Refreshing sprays in the bathroom on top of the toilet accessible to children. 5. Laundry room opened with detergent bottles in the floor accessible to children. 6. Loose cable from a security camera in the daycare room accessible to children and representing chocking a hazard. 7. Unlocked kitchen drawer with knives in it. 8. Broken shades in the living room representing an entrapment hazard.
Educator did not have PQR handy.
There’s no curriculum evidence. Only child present was observed watching TV during the visit. When asked about activities for school age children, Educator stated that they read.
Licensor observed 2 uncovered electrical outlets in the daycare room.
Licensor observed the following health risks: 1. Loose cables accessible to children in the dining room and daycare room. 2. Two uncovered electric outlets in the playroom.
