1. The educator did not have an accurate children’s attendance records, children’s in/out times were not documented at the time of arrival/departure as required. 2. The educator did not have an accurate assistant’s attendance records. The assistants did not signed in/out for the first week of Nov, 2023 and did not signed in/out for the wee of Dec. 11/2023.
One child’s record review did not have evidence of lead screening.
