Child #8- no physical description, Child #9- developmental history section is incomplete, general permission section is not signed, no date on physical exam, annual update is expired (7/29/22).

Licensor observed weekly attendance and it was not accurate. Information for the day was empty upon licensors arrival.

Licensor observed the following indoor hazards for children: 1. One unlocked kitchen drawer with two knives in it. 2. Two uncovered electric outlets in the kitchen 3. Laundry detergents in the bathroom floor accessible to children 4. Clorox disinfectant spray in the sink area accessible to children. 5. Two fans in the living room floor accessible to children. 6. Disinfecting wipes on top of childrens cubby.
Licensor observed 3 children with outdated progress reports and 4 children with no progress reports on file.
This investigation inspection recorded no violations or advisories.
Licensor reviewed the incomplete Children's Records Checklist submitted by the educator on 4/21/21, and a revised version submitted on 4/27/21, and observed the following: 1) two children CPR/ First Aid medical consent released forms were not up to date.

There are two exit doors: 1. Front door (leading to living room) 2. Back door (accessing from the kitchen and leading to backyard). Licensor observed boxes piled up in the front making the exit inaccessible.

Educator stated she removed the fences from her backyard space since she’s in the process of moving. Children now have access to safety hazards: trash, toys piled up, big pieces of wood, trash barrels, uncovered grill, mirrors.
Licensor observed toys and equipment in boxes since Educator is moving. Children had no access to toys.
