1. Educator stated that there is a firearm in the house and have not provided written communication to parents about this. 2. Educator stated that there was a change to household composition (a person moved out of the house) it was not notified in writing to parents, and transaction was not submitted through LEAD portal.

Educator stated that she has not completed any professional development after getting licensed in July 2024.
