1. The licensor observed that the smoke alarms battery replacement in the daycare room was removed and left with the exposed wires. 2. Two smoke alarms in the dining/living room kept beeping although the educator changed their batteries.

The educator did not have a parent handbook available at the time of this visit.
Six children record reviewed were incomplete, they did not have copy of the emergency cards and did not have evidence of current progress reports.
1. Two children records reviewed did not have progress reports every 3 months as required for children with special needs. 2. Two children records reviewed did not have any progress reports conducted annually since 2023 as required. 3. Two additional children’s records reviewed had expired progress reports from 4/8/2024, no new progress reports was documented at the time of this visit.

1. The educator has 2 coaching gun and caulking glue accessible to children on the secondary exit. 2. There are accessible tools, nails, tape, boxes, a shovel clutter and trash/recycled in the secondary exit door on the first floor.
