1. Upon arrival to the home the licensor observed that the educator was out of ratios by supervising 8 children alone, 7 of the children present were under the age of 5 years old. 2. The educator was exceeding the capacity of 6+2 school age children when she was alone supervising 8 children but only 1 school age was present.

The licensor observed that the educator had her {REDACTED} staying at the home while she was on vacation to care for 2 participating children. The {REDACTED} does not have a background record check (BRC) run to be authorized to be on the premises during daycare hours.
The educator did not provide appropriate supervision when the educator left a certified assistant approved for 6 children, alone to supervise 8 children, including 2 participating children present in the home. The assistant had 6 children alone in the basement and the 2 participating children were on the first floor under the care of the educator’s {REDACTED}, an unapproved caregiver.

The refrigerator temperature tested 20*F degrees. Thermometer er appear not to be working.
On 3/23/2023 during a conference call with this licensor and the FCC supervisor, Karen Brown, the educator said that her {REDACTED} was coming on 3/27/2024 to stay with her for 3 months. Upon arrival to the home this licensor observed that the educator’s mother was in the home cooking lunch in the kitchen. The educator’s {REDACTED} was already in the home since 3/16/2023. The educator provided misleading information regarding a new person on the premises, to the licensing staff prior to this visit.
Licensor did not observe sufficient quantity and variety of developmentally appropriate learning materials for young children (soft blocks, legos, wood puzzles, play-dough, large crayons, finger paint and manipulatives).
The exit from the basement to the first floor of the house is not labeled.

The licensor observed that the new constructed back porch has a fence that is been installed and it has not finished. (This area edge is as high as 21 inches above ground).
The water temperature tested 126.3*F degrees exceeding the maximum water temperature allowed per regulation.

1. The educator has house siding panels and equipment in boxes accessible to children in the backyard. 2. The educator has peeling paint and cracked cement on the backyard cement play area accessible to children. 3. There was a water hose on the ground, posing a potential tripping hazard for children in the backyard cement area. 4. There is a rug rolled up and standing up against the porch posing a falling hazard on children. 5. There was an old dissembled crib, a plastic canopy and gas tank accessible to children in the back porch

There was accessible Dish latino cable and TV cables hanging low and accessible to children in the living room.
Front door exit has accessible ice salt machine, bags ice melt salt and shovels accessible to children.
Licensor observed the following safety hazards: 1. Cement column in the basement is not completely padded. 2. The trash container in the bathroom is not lined. 3. The stairway from the first floor of the house to the basement is not well lit. This represents a safety hazard.
The playpen used as a napping material for the infant has holes on the net and the bottom board of the mattress is broken.

The educator has multiple pieces of wood (2x4) at the front yard posing accessible to children. 3. The educator has a water hose on the floor of the front entrance posing a potential tripping hazard.
