Licensor observed the attendance record for the week. It was observed that attendance was not accurately documented. For the 6:30 am to 4:15 pm shift, it was missing attendance for Thursday and today. For the 4:30 pm to 6:00 pm shift, attendance was not documented for Wednesday and Thursday

Licensor observed water temperature was 130*, exceeding the limit of 120*F.
Licensor observed the record for every child enrolled and the following was noted: Child #1- ok Child #2- missing date of admission, lead screening and transportation plan. Child #3- missing parents daytime phone number, drop off in transportation plan. Child #4- ok Child #5- missing parents daytime phone number, and drop off in transportation plan. Child #6- ok Certified assistant information needs to be included in permission section.

Although the Licensor observed a parent handbook created by the Educator, it must be updated since: 1. The daily operations schedule is outdated, since it does not indicate that the program closes at 8:00 pm. 2.Indicates that the program does not use an assistant. The program has a certified assistant who runs the shift until 8:00 pm. 3. Escape routes are not identified in contingency plan. 4. Does not indicate what she's going to do with the other children in the event of a missing child.
LIcensor observed the record for every child and the following was noted: Child #2- parents daytime phone number section is empty. Child #4- outdated record, outdated physical exam and lead screening. Child #6- missing lead screening

Licensor observed more than 10 hours of professional development for Educator. CA-6 has only 1 hour of professional development.
Indoor: Licensor observed extension cord accessible to children. Outdoor: 2 slacks of wood in the fence: 1 was missing and another one was loose.
