The licensor observed that the assistant, {REDACTED} went to the first floor to open the door leaving the 3 children unsupervised alone on the second floor while the educator open the back door to the licensor.
Licensor observed the following outdoor safety hazards: 1. The outdoor play space is not gated and the children can have easy access to {REDACTED}and {REDACTED} St. 2. Licensor observed a granite counter top leaning against the building in the driveway. The granite counter top is not secured and represents a hazard.
Licensor observed peeling paint on the frame of the door on the first floor. This door is part of the front entrance and evacuation route.
