Licensor observed the intake for every child enrolled and the following information was missing: Child #1- missing date of admission, schedule indicates child attends Monday and Tuesday, last progress report- 5/27/21 Child #2: missing consent for CPR and FA signature (it is only dated), missing progress report. Child #3: Scedule indicates child attends Thursday and Friday, missing progress report.

1. One child’s record reviewed was expired. 2. Two children’s record reviewed had an expired physical 3. Two childrenn’s record reviewed did not have evidence of {REDACTED}. 4. Three children’s records reviewed did not have the educator’s name on the general permissions or identified places to go.

The educator has not been documenting professional development training hours. (PQR #9551326 exp 1/24/20)
A review of her attendance records from 9/2019 to 1/2020 shows a parent of two children signing her children into and out of the program for periods of time as little as two minutes in care. The educator confirmed during this visit that she allows this parent to complete the attendance with times of arrival and departure, but these two children never actually stay in the program on those days. The educator did not have accurate attendance record when she allowed a parent to sign-in her children although the parent was not dropping off her children today.
