For the period from August 10, 2023, to August 9, 2024, no evidence of professional development was observed. Before August 9, 2026, the educator needs to complete 3 hours of training to meet the requirement of 10 hours of training per year.
Licensor observed the record for every child enrolled and the following was noted: Child #1- permission section is incomplete and no transportation plan is available. Licensors observed chipping and peeling paint on the front walls, and two plastered walls in the daycare.
Licensor observed attendance log for the day and it was not accurately documented. It had 8 written names without time-in information. Eleven children were present at the time of the visit.

Educator has 7 children enrolled and has 6 children records. Missing intake for 1 child.
1. Water temperature was 125*F, exceeding the limit of 120*F 2. Lid in bathrooms trashcan is broken. 3. Loose hanging tv cable in bedroom that will be added. 4. Uncovered outlet in the bedroom that will be added.

ALthough Licensor observed a parent handbook it was not updated with current license capacity (8), it indicates Educator is not working with an assistant and does not indicate current daycare location (basement).
Licensor observed the record for every child enrolled and the following was noted: Child #1- emergency card is empty, developmental history is not signed by parent; permissions section does not say Educators name. Child #2- transportation plan is incomplete, missing schedule of days and times in care, missing physical description. Child #3- missing date of admission, missing Educators name in general permission section, missing lead screening. Child #4- does not attend on the schedule indicated in the record, annual update is from 3/15/22. Child #5- missing IHCP (child has a diagnosed allergy), annual update is from 6/22/21. Child #6- Annual update is from 1/28/22 Child #7- emergency card information is empty, permissions section does not say Educators name. In addition, all children record are missing Asistants name.

Licensor observed first aid kit and it was missing the tape.
Licensor observed a trampoline in the outdoor space.
Although Licensor observed a written emergency preparedness plan it was not based on the 1st floor location.
During the visit, Licensor reviewed 3 random records of the children currently enrolled, the following was observed: 1) a child was missing a physical exam on file.

During the visit, the Licensor observed the refrigerator temperature was below (28 degrees F) of the required temperature at not less than 32 F degrees not more than 40 F degrees.
During the visit, no children were present. However, Licensor observed the educator has 4 children currently enrolled under {REDACTED}. years old since October 2021, exceeding the no more than 3 children under {REDACTED} years of age, including at least one {REDACTED} who is walking independently.

Licensor reviewed the Children's records checklist completed and submitted by the educator on 12/2/21. Additionally, 4 random records of the children currently enrolled were reviewed during the visit, the following was observed: 1) as child was missing the date of admission in the enrollment packet. 2) two children were missing the schedule of days and times in care in the enrollment packet. 3) two children were missing copies of the physical examination on file. 4) two children were missing immunization records on file. 5) three children were missing {REDACTED} results on file.

This is a program currently licensed for 10 children. Licensor observed 11 children at the time of the visit (10 childcare children and the {REDACTED} child of one of the CA-6).

11 children were present at the time of the visit, exceeding the license capacity of 10 children. The Educator with the highest qualifications was certified for 8. The Educator (who is the person certified for 10) was not present.
Licensor observed an industrial floor fan in use in the daycare area #2.
During the visit, Licensor Jayssyka O’Farrill observed a {REDACTED} napping face down inside a playpen. CA-8 acknowledged to have place the {REDACTED} face down on multiple occasions and not having written documentation of the {REDACTED} rolling over. The playpen had a plastic mat on top of the playpens mat covered with a loose quilted blanket. In addition, {REDACTED} was observed wearing a bracelet without written consent from parent/guardian

Licensor observed that assistants file: 1. Has an expired PQR ( {REDACTED}, exp.: 10/13/22). 2. Has no attendance for assistant working hours and Educator stated that assistants works every afternoon.
Although Licensor observed fire drills documented to be happening monthly, they are occurring in the morning.
Licensor observed water temperature was 125.6*F at the time of the visit, exceeding the limit of 120*F.
Educator has no emergency list posted.
Although licensor observed 30 training hours, they were all from 2022. Educator did not complete 10 training hours per year as stated in regulations.
Space (indoor / outdoor hazards) - 1
Licensor reviewed the Children's records checklist completed and submitted by the educator on 12/2/21. Additionally, Licensor reviewed the progress reports of the children currently enrolled, the following was observed: 1) four children were missing progress reports.
During the visit, Licensor observed despite educator having a thermometer inside the refrigerator, the thermometer was not properly working and marked a temperature over 80 F degrees, exceeding the no less than 32 F degrees no more than 40 F degrees required.
No indoor hazards observed. However, Licensor observed the following outdoor hazards: 1) a gap in the fence presenting an entrapment hazard for children. 2) some areas of the metal link chain fence were broken, making the fence non-sturdy and in need of repairs.
