This monitoring visit inspection recorded no violations or advisories.

Licensor observed the record for every child enrolled and the following was noted: Child #1, CHild #2- missing assistants name in permission section Child #3, CHild #6, Child #7, Child #8 and Child #9- have old assistants name in permission section
Folding mats are stored touching each other.
Although Licensor observed a parent handbook it was missing license capacity, whether or not an assistant works in the program, exits in the emergency plan.
Licensor observed toothbrushes are not air dried.

During the visit, Licensor reviewed the 6 records of the children that were present, the following was observed: 1. a child enrollment packet and {REDACTED} results were not up to date. Additionally, the child was missing the transportation plan in the enrollment packet. 2. two children were missing certified assistant {REDACTED} information in the general permission and Medical treatment. Additionally, the children were missing evidence of physical exam and {REDACTED} results in the file. 3. a child was missing certified assistant {REDACTED} information in the general permission and Medical treatment. Additionally, the child was missing evidence of physical exam. 4. a child enrollment packet was not up to date. Additionally, the child was missing certified assistant {REDACTED} information in the general permission and Medical treatment in the packet. In addition, the child was missing a physical exam on file.

During the visit, educator stated she is doing the toothbrushing routine. Licensor observed despite educator having toothbrushes available, they were not individually labeled with children’s names.
During the visit, the educator stated certified assistant {REDACTED} has been working in the program for two weeks. However, Licensor observed the certified assistant FCC license was not posted as required.
During the visit, Licensor observed the following: 1. Educator did not have attendance records of the 6 children that were present during the visit for today 4/25/22. 2. The educator stated the last day certified assistant {REDACTED} worked in the program was Friday 4/22/22. However, the educator did not have evidence of assistant attendance records.
During the visit, Licensor observed the following hazards accessible to the children: 1. Indoor hazard: 3 weights on the floor and an exercise rope in the dining area presenting a tripping and strangulation hazard. 2. Outdoor hazard: a large black plastic bucket that was filled with water.
