During the visit, Licensor reviewed the children's records checklist completed by the educator, and three random records of the children currently enrolled, the following was observed: 1) a child was missing an up to date physical exam. Additionally, the child was missing {REDACTED} results.

During the visit, Licensor reviewed the educator and Regular assistant {REDACTED} records/files, and observed the following : 1) no evidence of completed professional development hours on file for assistant {REDACTED} as required.
During the visit, Licensor observed a single green/red slide (31 inches) in the outdoor space which measured above 18 inches, and the educator did not have a fallzone as required.

The educator did not have current progress report for 8 children enrolled. She was using Ages and Stages questionnaire.
1. The educator did not have an accurate children’s attendance for the day of the visit. The children were not sign in at arrival or departure times. 2. The educator did not have an accurate assistant’s attendance for the month of January 2020.
