The licensor observed one of the children inside a bedroom not approved by EEC for child care. Child care must be provided only in space approved by the Department.
The licensor reviwed 3 of the children progress reports. The 3 progress reports are due: 2 of the children were due July of 2022 and one of the children progress report was due December of 2022.

Licensor observed, the educator did not document the children's arrival and departure times for today 3/11/2020. Additionally, Licensor observed the educator updating the daily attendance during the visit.
The water temperature was 130 F degrees, exceeding the 120 F required.
After the review of four random children's records, Licensor observed the following: 1) four of the children were missing the child's physical description in the enrollment packet. 2) four of the children were missing days in child care in the enrollment packet. 3) a child was missing departure time. 4) four children were missing the {REDACTED}'s information on the General permission n the enrollment packet. 5) four children were missing the {REDACTED}'s information on the CPR and FA medical consent and release in the enrollment packet. 6) two children were missing up to date physical examinations in the files. 7) three children were missing evidence of immunization records. 8) a child was missing the {REDACTED} results in the file. 9) a child's enrollment packet/annual up date was missing the date it was sign by the parent. 10) two children were missing up to date enrollment packet/annual update.
Refrigerator Temperature
The educator has been providing evening care until 11 pm without the approval from EEC.
The educator did not have sufficient toys and materials. She said that materials were stored away.
The children’s blankets were touching, stored in a pile.
The water temperature exceeds the maximum temperature of 120*F degrees allowed per regulation.

The licensor observed a child enrollment record not complete. The enrollment form did not have: 1. The front page of the enrollment with child's name, address, etc,. 2. The development page history. 3. The sleeping habits was imcomplete. 4. The general permissing was not filled out.
The program was not within its licensed capacity, when the educator had 11 children present at the same time. The program is licensed for 10 children.
The licensor observed 2 children enrolled at the program who have {REDACTED}. The educator did not have the individual health care plan for the 2 children. The educator and assistant did not use good judgment since they were caring for 11 children when the program is licensed for 10 children.
The licensor did not see evidence of curriculum routine or structure. At the time of the visit: 1. Three of the children were in the living room with a phone watching a program. 2. Four children were napping, 3. One child was sitting in an {REDACTED} seat. 4. One child was inside a {REDACTED}. 5. One child was playing with some toys inside an {REDACTED} seat. 6. One child was in one of the bedroom with the educator's assistant. The assistant then went to the kitchen with the child and started watching something on the phone with the child. During the visit, 3 children were picked up.

After the review of progress reports for the children currently enrolled, Licensor observed the following: 1) four progress reports were not completed. 2) two progress reports were not up to date.

Licensor observed the following indoor hazard accessible to the children: 1) an uncovered electrical outlet in the living room area. Note: The outdoor space is not approved. The educator stated she takes the children to a nearby park.
During the visit, Licensor observed a child who opened the door of an unlicensed bedroom and went inside. Additionally, Licensor observed two other children who attempted to open the same bedroom door. Furthermore, this being the third time, Licensor observed another child trying to open the door to the unlicensed bedroom. Note: License spaces included: 2nd Floor: -kitchen -daycare room -family room -living room
There was a plugged in air freshener in the family room accessible to children.
