The following indoor hazards were observed: 1)In the educators bathroom the bathtub had 5 different bottles of shampoo and conditioner accessible to children at the time of the visit. 2) A Lysol spray in the bathroom shelf was accessible to the children. 3) A cord coming from a radio was accessible to children at the time of the visit. 4)A brown plastic box was leaning against the kitchen wall posing a tipping hazard. 5). The licensor observed the following hazards in the 2nd exit nearest from the kitchen/dining room: 1) Two bottles of chemicals animal repellant and weed and grass chemical were accessible to children at the time of the visit.

The licensor observed the following from the educators first aid kit that was missing: 1) scissors 2)non-latex gloves 3)One emergency card was missing 2)One outdated emergency card 3)Two incomplete emergency cards.

The licensor observed the following when reviewing staff records: 1)The educator was missing Certified assistants for 10 her attendance records. 2)And for educators certified assistants for 6 she only had his license in his staff records (missing medical form, PQR, attendance record, professional development.)

Licensor observed the record for every child enrolled and the following was noted: All children records have incomplete developmental history and are missing assistants information on permission section. Child #1- outdated intake Child #2- Child #3- no physical description Child #4 and Child #5- no physical description, no signature on parent handbook, no transportation plan, general permission section is empty. Physicians information is incomplete Child #6- no signature on CPR/1st aid permission, missing physical exam, lead screening and v=inmunizations.
The licensors observed that in the daycare room one standing shelf was not secured to the wall at the time of the visit. Educator does not have sufficient materials for the license upgrade to 10 children that she is requesting for.
The licensors observed the educator completing a diaper change; however, handwashing procedures were not followed. The educator did not wash the child's hands or her own after diapering routine per regulation 7.11(10)(c).

Licensor observed the record for every child enrolled and the following was noticed: Child #1- general permissions section is not signed Child #2- missing lead screening Child #3- ok Child #4- missing parents daytime pone number, missing physician/health care information Child #5- Educator did not have childs paperwork (registration, physical, inmunizations). According to Educator today was childs first day attending the program.
Educator did not have evidence of having complete 10 hours of professional development. She had two training certificates adding 7.5 hours. Educator indicated that other training certificates are in her car. Educator is not documenting professional development hours into her PQR.

Although Licensor observed 1 playpen and 5 mats; mats were not stored in a sanitary manner (they were not individually stored).
