1. The interior stairway from the basement has wide space approximately 12 inches in between railings, that are an entrapment hazard. 2. The educator has low stairs fence/railing at the top of the interior stairway that is 2’11” height, posing a potential falling hazard for children. 3. The educator needs a gate at the bottom of the interior stairway in the basement.

1. There are 3 unlocked drawers and a cabinet door with cleaning supplies, knives, shedders, food scissors, a gas lighter, batteries, etc., accessible to children in the kitchen on the first floor. 2. There is an unlocked drawer with charging cables accessible to children in the lower level.
