1. The educator has a broken playpen for {REDACTED}. The educator acknowledged that she is placing the {REDACTED} enrolled on a crib’s mattress on the floor because the playpen is broken. 2. This licensor had a {REDACTED} year old sleeping on the sofa. 3. There was a broken mat with the exposed foam.

The following finds were observed in the home. 1. Trash can in the bathroom was not lined with plastic bags. 2. The indoor space was clutter by evidence of clothes, blankets, bags with items and toys in the living rom.
1. This licensor felt that the room temperature was very cold. Children were using jackets in the home. The home thermostat had a 65*F degrees temperature and felt lower. The room temperature regulation was discussed with the educator. 2. The indoor space was not well lit as required by regulation. The indoor space was to dark. This licensor was unable to see a child laying down of the living room floor in and some children were already awake.
The educator did not have evidence of curriculum or evidence of age appropriate activities.
