Licensor observed the record for every child enrolled and the following was noted: Child #1- no physical description, no address for emergency contacts, incomplete developmental; history Child #2- no address for emergency contacts, incomplete developmental history Child #3- incomplete date of admission, incomplete transportation plan, outdated IHCP, outdated enrollment packet Child #4- outdated enrollment packet, no IHCP on file and child has diagnosed allergies Child #5- incomplete physical description, no address for emergency contacts, incomplete developmental history, outdated general permission section and CPR/First Aid permission section Child #6- no physical description, incomplete developmental history, outdated general permission a section and CPR/First Aid permission
Although diaper change was not observed, Educator stated that she has no disposable barrier available.
Water temperature was 125*F exceeding the limit of 120*F.
Licensor observed the following materials accessible to children: Library, Dramatic play, musical instruments. Licensor also observed dolls, cars and other toys mixed up in different buckets. Educator had materials stored in a room that’s not part of the license, hence making materials inaccessible to childen: art materials, puppets, puzzles, blocks.
This change of location visit inspection recorded no violations or advisories.
Bathroom counter is not locked allowing children to have access to toiletries.

There is a five shelves shelving unit not secured to the wall. Two other shelving units are not secured to the wall.
