Cited finding
During the visit, the following was observed: 1) Educator had an infant under 12 months and a child of school aged (under 12 yrs old) present. Educator did not have records with the require information available at the program for both children. 2) Licensor reviewed the records of the 2 children currently enrolled, including a child that was present. The following was observed: -CHILD #1: emergency contact information was incomplete in the enrollment packet. -CHILD #2: emergency contact and doctor's information were incomplete in the enrollment packet. Additionally, the child did not have immunizations information in the file.
