The following hazards were observed at the front and secondary this visit: 1. There are appliances and a light electrical wire accessible to children at the front entrance. 2. There is a broken AC equipment in the secondary exit. 3. There are construction materials and cleaning supplies at the front foyer and entrance. 4. There are no “EXIT” signs posted at secondary exit.

Licensor observed two pack and play mattresses located in the daycare room area. Both mattresses were observed without the fitted sheets as required at the time of the visit. She did have two {REDACTED} in care at the time of the visit but they were not sleeping at the time of this visit.
One [redacted] child record reviewed has an incomplete enrollment package, and according to dates recorded on forms the record has an expired emergency card and forms.
