Licensor observed there was one smoke alarm missing in the living room area. Educator stated she is waiting for [redacted] to put it on. Also the educator did no have documentation of monthly alarms tests from January to October 2019 .

Progress Reports
Licensor observed the following after reviewing [redacted] children's records: 1. Children missing completed enrollments packets. On the files educator had the food program enrollment form. 2.Child [redacted] missing acknowledgement of parent hand book ,parental visit notice and missing developmental history 3.Child [redacted] has incomplete general permission 4.Children [redacted]and [redacted] missing up to date physical examinations 5.Children [redacted]and [redacted] missing lead screenings results.
The educator did not have CPR and First Aid certification available when requested by the Licensor. Educator stated that her coordinator from the agency she is affiliated with, [redacted] took it with her.
The first aid kit is missing the following: 1.Mask/Mouth Guard 2.Emergency cards of all the children
Educator had a parent handbook template provided by the agency she is affiliated with. Educator stated that [redacted] is supporting her with this. Educator also stated that she already send parent handbook home to be sign, but does not have them back yet.
Although educator has toothbrushes, they are not properly labeled with children’s names.
A [redacted] child who arrived during the visit, missing arrival time.
Licensor observed the following indoor hazards accessible to the children: 1. Window Air conditioner on the floor with loose cable. Licensor observed a slide outside without a fall zone. Note: The educator stated that children do not use the slide and she does not let them go to this area of the backyard.
