1. Two children records reviewed had expired enrollment papers. 2. Two children’s records reviewed did not have evidence of Lead screening on record. 3. One child record reviewed had an expired physical.
The educator did not have current progress reports on file.

The following hazards were observed by this licensor during the visit: 1. There were long wood panels with protuberant nails accessible to children. 2. The backyard shed was open with a grass mower machine and other items accessible to children. 3. There were also 2 metal ladders, a water hose on the ground, a wheel barrel up agains the shed and car roof rack standing up-right posing a potential hazard for children.

The educator has 4 dogs. 1. The educator did not have vaccination documents for 2 of the dogs. 2. The dogs were enclosed in the first floor bathroom and stepping on dog fecal matter which is not a sanitary manner.
