The following hazards were observed at the time of this visit: 1. There are 2 uncovered ice melt buckets accessible to children at the front entrance. 2. There is a large play wood panel standing upright and accessible to children at the primary entrance. 3. There are multiple pieces of wood, broken drywall pieces on the ground at the secondary exit pathway accessible to children. 4. There is a large mirror, a fan and uncovered table grill accessible to children in the secondary exit.
