1. A metal storage shelf unit measuring approximately 6-feet in height was not safely secured to the wall, presenting a potential tipping hazard inside a classroom space. 2. Electrical cords longer than 6 inches were not safely arranged within a classroom space and in a hallway space area, accessible to children.

1. At the time of the visit, a staff's personal belonging (purse) was stored inside an unlocked storage cabinet, within reach of children. 2. At the time of the visit, Clean Cide Disinfectant wipe product, indicating "Keep Out of Reach of Children" was stored on top of a bathroom shelf within reach of children. 3. Trash bags were stored underneath a countertop, accessible to children as the safety latch was not properly working.

An educator's personal belonging (purse) was stored on top of a small storage shelf, within reach of children in a preschool classroom.
Personal items intended for individual use by children, (two bottles) were not labeled with the name of the child for whom they were intended.
Electrical cords from electronic devices/extension cords longer than 6 inches were accessible to children within a preschool classroom and hallway area.
A current consent for over-counter medication was not available for review for a child requiring Benadryl medication.
Equipment is clean, safe, secured, and poses no hazard.

Materials/supplies stored inside plastics storage bins, were stacked 4-5 feet high inside an unlocked storage shed, presenting a potential tipping hazard. 3. A storage shed reflected three screws to be exposed from a broken door handle.

Additional sanitizing products were stored inside an unlocked shelf, accessible to children. 2. 1. Bi-monthly observations were not available for review in two staff record files. 2. Documentation of a physical examination and MMR immunizations were not available for review for a staff member hired on 2/5/2024.
A staff member did not wash their hands and a child’s hands following diaper changing procedures.
One child's arrival time was not properly documented in one classroom. Daily attendance reflected five children; however six children were present.
