Building Inspection and Fire Inspection
At the time of the visit, a review of two staff's record files reflected the following. 1. Documentation of bi-monthly staff observations, staff information form, employment applications/resume, documentation of an interview and verification of two references were not available for review in a staff’s record file. 2. Documentation of a current physical examination was not available for review in a second staff record file.
1. The program's building inspection reflected expiration date of 2/8/2025. 2. The program's Fire Inspection issued on 2/1/2024 was found to be outdated as of 2/1/2025.

Outdoor Space - R Apr 2025
1. Various safety lock mechanisms intended to secure drawers and counter cabinets appeared to be broken/damaged, therefore the following items were made accessible to children: adult size scissors, gel chaffing fuel, electrical cords, lighters and cleaning products (disinfecting wipes). 2. Kitchen barrier (door) was not safely secured and was kept opened through the duration of the visit and made accessible a staff’s personal item (purse) which had been placed on top of the countertop. Additional adult size scissors were stored inside a caddy which was placed on top of the countertop. 3. A second staff's purse was placed on top of a child size table during outdoor time, which was accessible to children.

Arrival time for one child in a preschool room was not properly documented as the attendance record reflected 15 children, however 16 children were actively present for the day. Technical Assistance is being provided.

Physical Facility-Indoors
Toxic Substances
Bleach products were stored in an unlocked lower shelf cabinet which was accessible to children. In the bathroom used by children, cleaning products were stored in an unlocked cabinet. A lighter and matches were stored in an unlocked drawer in the kitchen space which is accessible to children. In the {REDACTED} classroom, Clorox wipes and Fabreeze air freshener products were placed on top of the kitchen counter and shelves within the reach of children.
The emergency evacuation observations log did not include documentation for exit route used and the number of children that exited for each of the practice fire drills conducted. Documentation for a December 2019 practice fire drill was not on file.
(1) child’s {REDACTED} medication was not stored in its original packaging and had an expiration date of 10/2019.
(2) children’s Individual Health Care Plans did not include a parent signature.
Toxic Substances
1. Documentation of an updated first aid certification was not available for review in two staff's record file, last certificate on file dated 9/13/2021, with a renewal date for recertification required prior expiration date, 9/2023.
A review of three children’s records reflected the following. 1. An updated physical examination was not available for review. Last physical on file dated 9/30/2022, valid for one year from date. 2. A child’s first aid and emergency medical care consent form did not contain a parent signature and the date in which the form was completed by the parent.
A review of three staff records reflected the following. 1. Documentation of bi-monthly staff observations and an annual staff evaluation for FY 2023 were not available for review. 2. A staff’s record did not contain documentation of a physical examination, MMR immunization records, verification of two references, documentation of an interview, and a program orientation checklist.
A bolt from a chain link gate located in the playground was found to be unhinged from the top right side. of the gate, appears to be loose.
A review of the program’s FY 2023 evacuation observation records (practice fire drill logs) reflected the following. 1. Practice fire drills conducted on 10/13/2023 and 11/12/2023 did not contain documentation related to the time of when the practice fire drill was conducted, the number of children evacuated, the exit route used and the effectiveness of the drill. 2. Documentation of a practice fire drill was not available for review for the months of January 2023 through September 2023 and for the month of December 2023.
(2) children's files did not contained medication consent forms for prescription medications.
