During the visit, the following was observed: 1. There was a plastic bag on the floor in the toddler classroom 2. There were coffee drinks and educators' purses on a counter accessible to children. 3. Electrical outlets within the reach of children were not covered.

During the visit, an outdoor heating unit was not secured away for the children.
The wall in the toddler room had small holes and flaking paint that needed to be patched.

The Lead Teacher overseeing the program reported that an educator in the infant room started employment a week ago and didn't undergo program orientation and training as required by EEC.
Ratios were not maintained when one educator was observed supervising five infants in the classroom. The program was cited for this same observation during a August 2023 visit.
The tiles in the bathroom were stained and need to be replaced. The children's bathroom sink was leaking.

Ratios were not maintained when one educator was observed supervising seven toddlers on the playground.
The sink was not washed and disinfected between hand washing after diaper changes and food preparation in the Infant Room.
An educator in the infant room didn't wash the infant's hands after diaper changing. The Educator used hand sanitizer after changing the infant's diaper and did not wash their hands with liquid soap and running water.
The preschool educator allowed two children to use the bathroom independently simultaneously. The educator did not exercise appropriate supervision of two children who were in the hallway bathroom unsupervised.

There was peeling paint in the Toddler classroom. The electrical outlet within reach of children did not have a covering that prevented access to the receptacle openings. Preschool children had access to a clugged toilet.
This investigation inspection recorded no violations or advisories.
It was observed that the infant's two classroom attendance was not accurate. The educators had 4 children signed in but 5 children were in attendance.
Outdoor Space - R Apr 2025
On the toddler playground, it was observed that the educators were not properly supervising the children. The children were playing with two sensory tables on the ground filled with water.
At the time of the visit, the preschool bathroom was being used by children. Several minutes after the licensor's arrival, a "Do no Use" sign was taped on the door. The Assistant Director shared that the HVAC unit was being worked on, and a block of ice fell through the ceiling into the sink. The bathroom shouldn't be accessible to children.

It was observed, that the infant/ toddler playground sandbox was broken and being held together with tape, and the grass needed to be mowed.
It was observed in Toddler three classroom that an educator did not complete the two step table washing procedures.

At the time of the visit, practice evacuation drills were not being conducted monthly with all groups of children and all educators from approved space.
During the visit, there was a bottle of cleaner on the sink within children's reach.

Through a review of IHCPs, one plan did not have the names of educators that received training addressing the medical condition, including administering the medication.
During a review of the Toddler I classroom, it was observed the a string of lights was unsecured, creating a potential hazard to children.
During the visit, there was a broke geared rail leaning against the playground fence, creating a potential hazard to children.
