At the time of the visit, children in the Infant/Toddler room were eating at the table and in high chairs without educator interaction or assistance, as required by EEC regulations.
In the Infant classroom, there was an open box of Cheerios, crackers, and other snack items that were not stored in clean, covered containers.
An Assistant Teacher was observed in the bathroom changing diapers without the direct supervision of a teacher-qualified staff person, as required by EEC regulations.
At the time of the visit, the changing table surfaces in both the Toddler/Preschool room and the Infant/Toddler classroom did not have protective coverings of adequate size to prevent children from coming into contact with the changing surface.

At the time of the visit, it was observed that educators did not clean and disinfect the diaper changing table between uses.
One staff file did not contain evidence that references had been checked or documented.
In the Toddler/Preschool classroom, an educator left the room, leaving one educator to supervise six children alone. The required child-to-educator ratio for a Toddler/Preschool grouping is 1:5, with one additional educator required when there are 6–9 children present.
The diaper changing surface was not washed and disinfected after a child had been diapered.
