During this visit, Licensor observed a mouse running around the kitchen multiple times; this presents as a sanitary concern for all children, especially for the infant that is currently crawling. Please have professional exterminators come to the home to begin pest removal process and provide documentation of the plan.

Educator has play structures that require a fall zone (there is one that has been grandfathered in). There was insufficient playground materials for the structures that require a fall zone. Please obtain more shock absorbent materials to ensure it is 9 inches deep and 6 feet out in all directions; confirm once completed and a follow up visit will be completed.
