No peeling or flaking paint (indoor and out)
A child's prescription medication was not stored in the container in which they were originally dispensed.
The outdoor deck/porch located on the first-floor level reflected peeling paint.
Documentation of an interview and verification of two references were not available for review for two staff members.

Administration
A list of all emergency and lifesaving medications was not posted in a manner that protected the privacy of each child in a {REDACTED} classroom.
1. Plastic trash bags were stored inside a bin drawer located in the restroom, within reach of children. 2. A staff’s purse was stored inside an unlocked closet in a {REDACTED} classroom within reach of children.
The staff meeting log reviewed on site did not reflect documentation of a staff meeting conducted for the following months; January, February, April, May, June, August, September, October and December of 2021. A review of the 2022 staff meeting log did not reflect documentation of staff meeting conducted for the months of January, March, April and July of 2022.

The emergency evacuation observations reviewed for FY 2021 and January through August 2022 did not include the exit route used.
The Health Care Consultant Agreement expired 11/6/2020. A Health Care Consultant is currently not available to the program for consultation as needed. Program is working on getting a new Health Care Consultant.
First Aid and CPR
Health Care Consultant Agreement
All staff's CPR and First Aid Certifications expired as of October of 2020. No educator is currently certified in CPR and First Aid present at the time of the visit. Program is working on getting scheduling training for all staff.
