Upon review of the program's School Age Staff Record Checklist, the following was indicated: -There was no physical listed for an educator. -There was no evidence of MMR for one educator.
The Staff Schedule for the program's School Age component is inaccurate and needs to be revised and uploaded to the LEAD document library.

Upon a review of the BRC Navigator system, one educator was listed as pending status. The Licensee of the program is aware of this and has previously reached out to the BRC Unit for assistance.
The program did not have a current Building nor a current Fire Inspection Certificate. The Building Inspection Certificate posted at the program expired on 8/31/22. The Fire Inspection Certificate posted at the program was conducted on 2/16/21.

During the visit, the program did have a Staff Schedule posted. However, this was not a current as a staff member had resigned in December.
During the visit, the following was observed: -In the Preschool room there is a large stained area on the floor near the block area, near the blue bean bag chair, and near the red child sized chairs. -In the Preschool room there is stained area under the kitchen sink baseboard cabinets. -In the Infant and Toddler class, there is a stained area on the floor where children are learning to walk.
The Building Inspection Certificate expired August 31, 2021.

Upon review of the Children's Record Checklist in the program's document library: -Five children's records were lacking current physicals and immunizations. -Six children's records were lacking results of Lead screening.
At the time of the virtual visit, the program had completed Evacuation/Fire Drill logs for the months of September and December but were missing documentation of drills in October and November.

The Program's last Building Inspection Certificate expired on 8/31/20. This has been extended due to a COVID delay.
Upon review of three children's files, one file lacked the required paperwork for a child with a (REDACTED). The Individual Health Care Plan (IHCP) and the Medication Consent Form were missing from the child's record. Consent and authorization must be renewed annually for administration of medication and/or treatment.
The Building Inspection Certificate expired on 8/31/2021. The Fire Inspection Certificate has an inspection date of 12/3/2020. During the visit, the Acting Director stated she is working in partnership with the local public school to have both inspections updated.
The program did not have evidence of a fire/evacuation drill during the month of September 2021.
Upon a review of three Children's Records, one is lacking a complete Individual Health Care Plan. The IHCP must include the name of the educators trained in the child's medical condition and the name of the person/agency that trained those educators.
The following was accessible to children: -In the Preschool room, a radio wire longer than six inches was hanging off a shelf in the block area. -In the Preschool room, an air conditioning unit on the floor next to the first window had wires and cords longer than six inches.
A review of medication stored at the program in the Preschool room indicated that a child's (REDACTED). This means the child would not have(REDACTED).
