Supervision of children
The wait time in the hallway for gymnastics was almost 10 minutes. The children were asked to sit on a bench and wait for the gym doors to open. Three Children struggled to wait, getting up running around, climbing the sides of the stairs case, running out of the hallway and yelling. Three staff struggled to keep the children safe and themselves safe from the upset children. During the Gymnastics time three children struggled to follow the direction given. They were made to wait in line for their turn on balance beams that were about four feet from the ground. The children were cutting the line, climbing on the beam with other children on the beam, under the beam hanging from it, going in-between the mats that created the fall zones, and climbing onto high beams that did not have a spotter or gymnastics trainer. While the gymnastics instructor gave direction for each activity the same three children would run away and needed to be caught by an educator. The three children were not provided Directions in a way they could understand them, which lead them into unsafe behaviors when the activity began.

A preschool child went into the bathroom alone and closed the door, the educators were not present. The child was out of site for about three minutes.
While observing the preschool classroom the Preschool Teacher had to raise her voice several times to get the children's attention. During gymnastics they repeatedly used a loud harsh tone with some of the children that were not following directions.
12 children were signed into the paper attendance, when 14 children were in the classroom. It was reported the program uses the brightwheel application to take attendance as well. 14 children were signed into brightwheel.
Two children who choose not to participate in gymnastics were told to participate or sit against the wall. The child was told if they did not listen they were going out to the hallway. The child was not encourage to participate and the desired behavior was not explained in a way the child could understand. The educators did not offer the child ways to self regulate and express themselves. The educators yelled across the gym at children trying to get them to listen.
Some children did not want to participate in gymnastics, alternative activities were not offered. The wait times in the gymnastics class were too long for some children, those children where exhibiting unsafe behavior, climbing up on balance beams alone, climbing under them when other children where on them, climbing and rolling around in-between the mats, and running around.
When walking into the playground the pathway down to the play area was washed away exposing wooden beams in the ground creating a tripping hazard. The wooden lining on the bottom of the fence was splintering and falling apart. The grass area of the playground had several large holes in the ground that children could trip on or get stuck in while running around. The black lining under the rubber much was exposed in several areas. The window near the staircase and the shed was broken. There was about a half inch hole with broken glass webbing around it.
There was peeling paint in the circle area.
On the day of the visit the Building inspection was expired.

Fall zones adequate depth of impact absorbing material
1.The little tikes playscape was missing a piece that held the structure together. The structure was being held together with zip ties. The slide was not connected to the base on the left side. 2.The rubber mulch was spread out so thin in several areas, the cloth underneath was exposed, resulting in a tripping hazard. 3.There was a broken plastic work bench next to the sandbox.
The pool inspection was expired on the day of the visit.
A child who required an IHCP did not have one.
The required fall zones did not have adequate depth of impact absorbing material.
Child Guidance & Interactions
IHCP for a child with a Epi Pen was expired. Educators were busy with children, licensor was not able to ask them about the IHCP or medication. The program director spoke about the IHCP. Program director was not aware the IHCP or Medication was expired.
The educators in the 4’s room were not being attentive and did not position themselves to be in close proximity of the children. When children needed guidance it was not being offered. The room was observed during a transition time. Both educators were supporting 1 or two children while the rest of the children were running and climbing, the educators did not address the children that were running or climbing.
There was a library area in the room with a few books. The area did not have a variety of books, furniture to sit on or any quiet manipulative or materials for the children.
Hand washing procedures were not posted a children’s eye level near sinks. Children were observed doing things that warranted hand washing and it was not encouraged by the educators. The educators did not wash their own hands after cleaning up from snack.

Space includes small quiet area
During the observation in the 4 year old room, a child was trying to run away from the teacher, the teacher then grabbed the child’s wrist to catch her and the child was trying to pull away from the teacher. Children were in transition to changing into their swim clothes. The children who were not changing had no teacher guidance. They were running around the room and one child climbed up on the table. There was very little teacher child interaction other then the teacher who was changing the children, she was interacting with the individual child while assisting them in getting changed.
The classrooms had very limited equipment and materials for the children. The interest area’s were not established, labeled or set up for children to make decisions of what and where they wanted to play. Curriculum was not evident in either preschool room. There were no visible planned activities. The materials offered in the PS rooms were limited and not all age appropriate. There was no evidence of linguistically appropriate activites or materials.
The 4’s room had cloth stapled to the toy shelves covering all the materials on the shelf.
The shelves in both rooms were not secured. Both rooms had dark spots, food, paper, chipped tiles on the floors. Both classrooms needed deep cleaning on toy shelves, bathrooms, cubbies and countertops. The three’s room had a broken wooden farm that was chipped and had sharp edges. There was a cord hanging down from a shelf in the reach of children. Two of the windows in the 2.9-4 year old room were broken.
The bleach and water solution was on the sink in reach of children.
Epi-Pen observed was over a year expired. The Epi-Pen was appropriately stored and readily available.
Children each had a cubby for their belongings, the children’s blankets, pillows, jackets, water bottles, bags and daily activities were all spilling out of their cubby touching other children’s belongs or on the floor.
This renewal - monitoring visit inspection recorded no violations or advisories.
On the day of the visit, the following items were identified as potential hazards: 1) In the bathroom there was a piece of wooden trim that was split. 2) There was rust accessible to children on the door jam in the bathroom. 3) The counter in the prep are was broken, with sharp edges. 4) The cabinet doors in the prep area posed a pinching hazard. 5) There was pieces of the tile floor near the cabinets in the prep area that were broken.

The program will upload a current fire inspection that will include an inspection of this room, with verification from the fire dept. that the room meets the code for Carbon Monoxide Detectors.

On the day of the visit there was identified areas of the classroom that had peeling or flaking paint that included on the door jam of the utility closet and the door to the storage closet.
