There were toilet brushes and plungers accessible to children in the preschool classrooms, the bathroom radiator was exposed in the brown bears classroom and the end caps were loose with sharper edges. The two sinks in the brown bear classroom were dirty with old debris.

Playground free from hazards; fenced if applicable
In the school age classroom there was tippable furniture through out the classroom.
During the visit there were a few areas that were identified throughout the center as needed attention due to being dirty, several of the sinks through out the center had gunk around the faucets and the caulking was dirty/moldy. One sink was rusting. The wall near a trash can was dirty with old spills. The heater in the bathroom in the preschool classroom is rusting and is rough to touch and could become a hazard to children. The heater in one of the classrooms had a part on it that was rough to touch that could be a potential hazard (please check this throughout the center and fix as needed).

Equipment is clean, safe, secured, and poses no hazard.
One of the climbers on the playground had a few identified hazards on it. There were three different plastic pieces on it that are broken and the coating on it is breaking creating sharp pieces that also pose as tripping hazards.

At the time of the visit there was a child who required an EpiPen for an allergy but there was only one EpiPen in the box despite the allergy plan calling for two. Plan says if symptoms do not improve a second dose can be given.

At the time of the visit there was not enough mulch under the school age climber, there was 8/7 inches in different locations. There was a wooden structure on the infant/toddler playground which has wood that is deteriorating throughout the structure and is a potential hazard to children. The sand on the preschool play structure measured at 8 inches during the visit.

During the visit there was a microwave in a classroom that was dirty with old food that splattered, the refrigerators throughout the program were dirty with spills that sat. There was a cord near a desk in one of the preschool rooms that was longer than six inches and accessible to children. There was a plunger in the school age bathroom that was accessible to the children. In the preschool classrooms furniture was stacked on tables while children were in the rooms, furniture should not be stacked in the classrooms while children are present. There was also a bike on top of a shelf in a preschool class that could fall and be a hazard to children. The caulking next to one of the sinks (in the preschool classroom) was dirty and molding. Also around the same sink the faucet had black substance around it. In the infant toddler classroom there was material along the wall that was peeling that needs to be addressed as children can peel the material and potentially put it in their mouths.
It was reported by a classroom teacher that fire drills are done quarterly but then the educator asked the director for clarification and the director reported that they are not always done monthly.

All three playscapes were used by the children. The playscapes did not have adequate depth of impact absorbing material. The Licensor observed a Little Tikes house without the roof, the integrity of the structure was not secure, it was shaky. The program immediately removed the structure. A Little Tikes kitchen area with a canopy was observed, while a child hung from the canopy and the structure came up off the ground. The program immediately removed the canopy.

The medication observed expired on 3/18/2023. The prescription on the medication, did not match the medication consent form provided from the doctor. The medication was appropriately stored. Medication administration was not observed.

No peeling or flaking paint (indoor and out)
While observing the diaper changing process the educators did not wash their hands after removing their soiled gloves. Children’s hands were not washed or wiped. The changing table paper was changed by a educator after the child was removed from the table, the educator did not wash their hands after handling the dirty changing paper.
The climbing structure with wood chips around it measured at 4.5 inches of impact absorbing material. The climbing structures with sand in the fall zones measured at 9 inches of impact absorbing material.
The wooden climbing structure on the Infant toddler playground was weather and could give children splinters. The wooden shed on the playground had broken pieces of wood and areas that the wood was splitting, leaving abrasive areas a child could get a splinter on or cut themselves on.
